you are probably misinterpreting something in the OG explanations; having modifiers both before and after a noun is most definitely not an error. (for another problem in which this construction is just fine, see the correct answer to OG12 #107.)

of course, the OG does contain a few SC answer explanations that are not quite correct, so maybe you saw one of those.
on which problem does the OG say that this is an "error"?

one of the Manhattan instructor BEN KU has given this reason to eliminate C :

the two modifiers of the Altar of Heaven ("having been constructed," a participial phrase, and "the oldest known altar," a modified noun phrase) are not parallel


C says : The Altar of Heaven, having been constructed at least as early as the Sui dynasty (A.D. 581-618) and the oldest known altar used in Chinese state religious practice, is more than 1,000 years older than a similar one in Beijing and

however "having been constructed" ,a participial phrase acts as an adjective and so is "the oldest known altar used" also acts as an adjective .so they should be parallel ?

"The oldest altar..." is a noun, not an adjective.

It's used as a modifier to provide further description/clarification of a thing (noun), but it's not grammatically an adjective. So it's not parallel to something that is actually an adjective.
