Salary History Question

The application I'm filling out asks for a professional resume with a salary history. Is it necessary for me to list this on the resume, or can I have a separate document with my salary history?

I'd rather put this in a separate doc as it will screw up the formatting of my resume.

Will it look bad if I don't include it on my resume even though it is listed under that section on the application?

Follow the instructions. I'm guessing you're talking about the Chicago application. If they ask for a salary history on your resume, put it in your resume.
