Both, actually. Do some at the end of a chapter, do a mixed set from all chapters at the end of a book, then as you finish more books, start trying mixed sets of questions from all previous topics. Not for every practice set, of course, or you'll run out of questions from the earlier topics. But you do want to make sure that you're mixing things up as you progress in your studies. The real test won't group topics or tell you that you're about to get a divisibility problem. You have to figure that out for yourself. :)
