Help, what do I do if I forgot the start dates of my past employment?

You can simply make a best guess at the dates. If you are a few days off or even a month off, then it won’t matter. The schools are not punitive. They are looking to ferret out those who are falsifying information and engaging in fraud, not those who are simple doing their best to remember arcane facts.
